SAVE TIME BY FILLING OUT YOUR FORMS BEFORE ARRIVING

Filling out forms on your first visit can sometimes be a time consuming process. We have simplified this procedure by putting forms online for you to fill out prior to your first visit.

If you carry dental insurance, we will be happy to process your claims for you. During your visit with us, we will provide you with a short form to fill out that will enable us to process your claims. We do not accept assignment of payment from your insurance company. We prefer to have your benefits sent directly to you. Our business staff is available to answer any insurance questions you may have.
